Below is a form that can be used for personal evaluation. At Apostolic Faith Tabernacle we will be using this form with the ministry staff to help them evaluate their preaching and teaching. However, this form could be used for Sunday School teachers or ministry leaders. The idea here is to reinforce the positive and have leaders look within to see how they can grow.
